Should an idle speed outside of spec be a cause for concern?

Not sure if this is petrol specific or not, but my service manual says that under heavy load my idle speed should be 720+/-50 rpm. My idle speed is basically always between 800 and 850, rock solid and not bouncing. Could this be a vacuum leak of some sort?

On smaller engines (either two stroke or turbocharged ones), it can lead to catastrophic piston damage if the leak is between the manifold and the engine, since the mixture becomes weak.

On a four cylinder car with injection I doubt it will make any difference to the engine, but you can spray liquid grease over the seals and if there is a leak, the engine speed will change.
